Informed Consent & Terms of Service for Herbal Energetics Products

The  practitioner acknowledges and agrees to the following terms regarding the purchase and use of Herbal Energetics tinctures:

Qualification and Application Process:

I understand that tinctures offered by Herbal Energetics are available exclusively to licensed and/or appropriately qualified Health Care Practitioners. I have completed the required application form and provided proof of my credentials. Upon approval, I acknowledge that I will receive a welcome email that includes the product and price list along with ordering instructions.

Intended Use and Regulation Compliance:

I acknowledge that Herbal Energetics products are sold solely for use within the context of a practitioner-patient relationship. I understand that these products are not intended for retail sale, do not possess Natural Product Numbers (NPNs), and that no health claims have been made or evaluated by Health Canada or the FDA.

Custom Formulas and Practitioner Discretion:

I recognize that all Herbal Energetics formula batches are custom made for individual orders. By purchasing formulas, I agree to the specified quantities included in my order. I understand that any dosing suggestions provided are ultimately at my discretion as the prescribing practitioner.

Liability Disclaimer:

I acknowledge that Herbal Energetics is not responsible for any adverse reactions or safety concerns that may occur with the use of its products. It is my responsibility to inform my patients/clients about the appropriate use of herbal products and to monitor their health and safety. It is also my responsibility to carry malpractice insurance when recommending herbal products in a clinical setting.

I acknowledge that Herbal Energetics does not test for gluten content in finished products and that none of the products carry gluten-free certification. Herbal energetics cannot guarantee allergen-free facilities and it possible that some small amount of cross-contamination may be present in raw herbal materials.

Refunds

I acknowledge that all refund requests must be made within 30 days from receipt of merchandise and will require the BIN# and proof of payment of the product.
